Incoming 6th Grade Health Letters & Forms

To the Parents of All 5th Graders,

 

The State of Illinois requires the following for entrance into sixth grade:

 

  1. A physical examination within one year prior to the first day of school. Physical Form
  2. A dental examination Dental Form
  3. Meningococcal Immunization
  4. Second varicella immunization
  5. Second MMR immunization
  6. Tdap Immunization
  7. HepB Immunization (series)

The enclosed physical form should be taken to your physician for use in recording the results of the examination and immunizations of your child.  The health history portion of the physical form must be completed and signed by the parent/guardian.

 

We would like the physical examination form turned in to the health office at your child’s school any time between now and the last day of this school year.  The completed physical must be turned in before the first day of school.  Attached for your convenience is a list of medical facilities that offer school physicals.  We would also appreciate the completed dental examination form be turned in before or at the beginning of the new school year.
